1. Download BLANK certificate

2. Print and Complete with Your Name, Credentials, and the number of credits you are claiming. Note - APP's and Physicians have different certificates. Make sure to print the correct certificate for your degree!

3. Make sure the number of credits you claimed in the evaluation survey match the number of credits you claim on this certificate.

4. Don't forget to take a photo for your records.

5. File wherever you file your CME credits!
